Guidance on Concessionary Travel 2008/2009

Guidance to Application Form: Concessionary Fares Scheme 2008/09

All those people who become eligible for a concession after the 1st April 2008 will have to complete the application form and follow the guidance below. Follow this link to assess your eligibility for this scheme.

The forms are available at Castle View for collection or you can download a copy by following the link at the bottom of this page. The form can be produced in large print, if required.

A leaflet detailing the terms and conditions of the new Concessionary Travel Scheme is available from the Council reception at Castle View, and an audio tape of the leaflet is also available, on request. 

If you are blind/partially blind, please get someone to fill the form in for you (the talking pages tape will explain it to you).

Important

• All those applying must be residents of the Borough of Oswestry (i.e. pay Council Tax to us).
• The application form must be completed in full.
• Please let us know if you ever change address at any time.

Signature

All forms must be signed by the applicant. For those unable to do so please seek advice from 01743 210336 or visit your local Council office.

Photographs

A passport-size colour photograph is required to accompany your application. You may supply your own or be photographed by our staff in reception at Castle View

• Please glue the photograph to the space provided on the application form, (so we can put this onto your new bus card).
• Do not stick the photograph with sellotape as it will not scan properly.
• Ensure that the glued photograph is fixed firmly to the application form. It will have to be done again if they become separated.
• Photographs should be no more than 12 months old.
• The only difference to a passport photograph is that you can smile if you wish.

Alternative to the Bus Pass

If you wish to forgo your bus pass, you must still fill in the application form but indicate you wish to receive the Senior Citizens Railcard by ticking the appropriate box. The cost will be £7 to any recipient for year 2008/09. This only applies to those who are 60 years +. For persons with an eligible disability, a voucher costing £4.50 - value £13.50, may be purchased from the Council and may be used towards the cost of a National Rail Card, (total cost £18.00). Rail Cards/Vouchers may be purchased from the Council main reception at Casttle View.

Delivery of Completed Application Forms

All completed forms must be hand delivered to the Oswestry Borough Council's Offices at Castle View, Oswestry, together with appropriate proof of eligiblity. Follow this link to work out what suitable proof you will require and the conditions of concessionary travel.

Important Terms and Conditions

Bus Cards remain the property of Oswestry Borough Council throughout their validity. If you cease to be a resident of Oswestry, you must return your card to Oswestry Borough Council, Castle View, Oswestry, Shropshire SY11 1JR.
